Supported Independent Living (SIL)

Everyone has the right to choose where they live, who they live with and the supports that help them live their life.

Community Care Foundation provides Supported Independent Living (SIL) supports across the Fraser Coast, assisting participants to live as independently as possible while receiving the level of support that meets their individual needs and goals.

Our SIL supports are designed around each person’s preferences, routines and lifestyle. We work alongside participants to create a safe, comfortable and supportive home environment where they can build skills, maintain independence and be actively involved in everyday life.

Our SIL Supports

Our experienced Support Workers provide personalised support that may include:

  • Personal care and daily living support
  • Assistance with household tasks and routines
  • Meal preparation and developing independent living skills
  • Medication support
  • Support to access the community and participate in activities
  • Building confidence and maintaining relationships
  • Support to achieve individual goals


The level of support provided is tailored to each participant, including the appropriate staffing arrangements and support hours required to meet their needs.

SIL arrangements may be available in a participant’s own home or within a Community Care Foundation leased property. Where required, our SIL homes could include general household furnishings to support participants to feel comfortable and settled.

SIL can be provided in a shared living arrangement with up to three participants. We take the time to understand each person’s preferences, support needs and compatibility when considering shared living options. Our focus is on creating homes where people feel respected, included and supported.

Short Term Respite

Short Term Respite (STR), provides participants with the opportunity to stay away from their usual home for a short period while receiving support in a safe and welcoming environment.

STR can provide opportunities to:

  • Experience a change of environment
  • Develop independence and confidence
  • Try new activities and experiences
  • Maintain connections and relationships
  • Provide families and carers with a planned break

 

Support provided during STR may include personal care, daily living assistance, community participation and activities that align with each participant’s interests and goals.

Independent Living Options (ILO)

Independent Living Options (ILO) support participants to explore and create a living arrangement that best suits their individual needs, preferences and lifestyle.

ILO focuses on the person’s goals, relationships, routines and support networks to develop a plan for how they want to live and what supports will help them maintain independence.

Community Care Foundation works alongside participants to understand what is important to them and identify flexible support options that may include:

  • Building skills and confidence to live more independently
  • Exploring different living arrangements
  • Developing informal and formal support networks
  • Connecting with community and local services
  • Creating routines and supports that align with individual goals


ILO is about creating a personalised approach that supports participants to have choice and control over their home and lifestyle.

MTA

Medium Term Accommodation (MTA)

Medium Term Accommodation (MTA) provides temporary accommodation and support for participants while they are working towards a longer-term housing solution.

MTA may be suitable for participants who need a safe place to stay while waiting for their long-term home or supports to be arranged.

During an MTA stay, participants may receive support with:

  • Daily living activities
  • Personal care
  • Household routines
  • Accessing the community
  • Building independence and confidence


Community Care Foundation provides a supportive environment where participants can feel comfortable while planning their next steps.

Our team works with participants and their support network to ensure supports are aligned with their needs, preferences and goals.

Who We Support

Our SIL, STR, ILO & MTA supports are available to participants who have the appropriate funding and approval within their NDIS or NIISQ plan.

Community Care Foundation also provides Specialised Disability Accommodation (SDA) for eligible participants.
