Everyone has the right to be included, connected and supported to participate in their community in ways that are meaningful to them.

Our Community Access supports assist participants to engage in everyday activities, build confidence, develop skills and pursue their personal interests and goals. We work alongside each person to understand what is important to them and provide support that reflects their individual choices, preferences and needs.

Community Access may include support to:

  • Attend appointments, community activities and social events
  • Participate in hobbies, recreation and leisure activities
  • Build relationships and connect with others
  • Develop independence and confidence in the community
  • Access local services, facilities and opportunities
  • Explore new interests and experiences

Our experienced support team focuses on creating positive, respectful and inclusive experiences. We recognise that every person is unique, and we support participants to have choice and control over how they spend their time and engage with their community.

Whether someone is developing new skills, maintaining existing routines, or simply enjoying activities they value, our goal is to provide flexible support that promotes independence, wellbeing and meaningful participation.

Finding the Right Fit

Community Care Foundation does its best to match Lifestyle Support Workers to Participants and their needs and requests. Our team will put you in contact with our Facilitators who conduct an intake meeting to discuss your Service planning needs. From there, Participants select the day(s) and time(s) they would like to receive support.
