Some people living with disability require more specialised support to manage complex health needs and live safely in their home and community. At Community Care Foundation, we understand the skill, consistency and clinical oversight needed to provide this support well.
Our team supports people with complex disability and high intensity support needs through personalised support arrangements designed around their health, wellbeing, preferences and goals. Whether support is provided in a person’s own home or through Supported Independent Living (SIL), our focus remains the same: delivering safe, capable and compassionate support that helps people live their lives with confidence.
High Intensity Daily Personal Activities under the NDIS involve specialised, health-related supports that require specific skills, training and careful attention to individual needs.Â
At Community Care Foundation, our registered nurse plays an important role in strengthening the knowledge and skills of our support team. Through relevant training, clinical guidance and ongoing oversight, we work to ensure our staff understand each participant’s individual support requirements and are equipped to provide the care they need.
Support is planned around the individual, with appropriate health professional input, clear procedures and attention to changes in a participant’s health or circumstances. This helps create a more consistent approach to care, with participant safety, dignity and wellbeing at the centre of every decision.
We recognise that people are more than their diagnoses or clinical requirements. The right support should enable choice, protect dignity and create opportunities for people to maintain their independence, pursue their interests and feel at home in their community.
Our approach brings together personalised support planning, appropriately trained staff, clear communication and collaboration with participants, families, guardians and health professionals. We take the time to understand what matters to each person and work with their support network to establish arrangements that are responsive to their needs.
For participants transitioning from hospital, or whose support needs have changed, we can also work with relevant stakeholders to coordinate accommodation, staffing and ongoing supports, helping establish a considered pathway into the community.
Delivering high intensity supports requires more than good intentions. It requires the right people, the right training and a commitment to quality, accountability and continuous improvement.
With more than 30 years of experience supporting our local community, Community Care Foundation combines experienced disability support teams with clinical guidance and established governance processes. We work collaboratively to manage risks, respond to changing needs and maintain a consistent focus on safe, person-centred support.
At Community Care Foundation, complex support needs don’t define a person’s possibilities. We bring the skills, coordination and care needed to help people live well.
If you’re looking for high intensity supports for yourself, a family member or someone you support professionally, we’d welcome a conversation about their needs and how Community Care Foundation may be able to help.
