  • Comprehensive staff performance monitoring & HR processes to promote professionalism throughout our teams – To ensure you are supported by not just anyone but staff delivering the high standard of support you should expect!

  • Financial compliance team to ensure appropriate & fair invoicing of participants for services provided, and all related documentation completed.
  • Focused on ensuring all different stakeholder & services working with our participants are also assisting our participants professionally, by:
    • Having an organisational team with high level knowledge understanding of NDIS and NIISQ roles & responsibilities of all stakeholders/ services working in the NDIS and NIISQ system

What does Community Care Foundation do to assist our participants, their families, and guardians?

  • Professional & Caring Support Teams Free Specialist Support to train & mentor staff and guide processes:
  • Qualified Nurse
  • Office Participant Support Team – facilitators to monitor & support facilitators teams. High level expertise in Supported Independent Living operations & general supports for complex participants
  • Quality & Compliance Team – including regular mandatory staff training, workplace health & safety (linked to SIL homes) and NDIS and NIISQ reporting compliance

Who we help?

This service is open to Participants of all ages, up to 65 years of age, and those with a range of needs with appropriate NDIS and NIISQ funding.


Finding the Right Fit


Community Care Foundation does its best to match Lifestyle Support Workers to Participants and their needs and requests. Our team will put you in contact with one of our NDIS and NIISQ facilitators who conduct an intake meeting to discuss your Service planning needs and support preferences. From there, we work collaboratively with participants to develop an appropriate support team and the days and times suited to participants.
