Community Care Foundation is a local charity built on care, connection, and community. For nearly 30 years, we have walked alongside people across the Fraser Coast — including Maryborough, Hervey Bay, and surrounding areas — supporting them to live rich, meaningful lives with dignity, choice, and belonging.

Many people in our community first knew us as Life Choice Wide Bay Burnett or Town and Country Community Options. While our name has evolved, our purpose has never changed. We remain deeply rooted in this region, proud of our local history, and committed to the people who call it home.

At the heart of everything we do is a belief that everyone deserves to be seen, heard, and supported as an individual. We work with people of all ages, supporting them in their own homes, in shared living environments, and within the community. Our services span NDIS supports, Supported Independent Living (SIL), Community Access, Respite and NIISQ‑funded services, all delivered with the same focus on respect, safety, and genuine care.

We know that no two lives are the same. That’s why we take the time to truly listen — to understand personal goals, routines, relationships, and what matters most to each person. Our experienced, compassionate team works closely with Participants, families, and support networks to create individualised supports that grow and adapt over time.

Community Care Foundation is made up of people who care deeply about the work they do and the people they support. Our staff bring not only skill and experience, but empathy, patience, and a strong sense of responsibility to the communities we serve.

As we continue to grow and evolve, our focus remains on building safe, stable supports and strong relationships that stand the test of time. We are proud of our past, driven by purpose in the present, and committed to creating a future where people feel supported to live the lives they choose — within a community that truly cares.
